Behavioral Health Services Facilitator w/Family Functional Therapy Assignment

Starting Hourly Pay Range: $32.87 -  $35.60


Job Summary
The Behavioral Health Service Facilitator ensures that the consumer’s Individualized Recovery Plan is coordinated, monitored, and designed to support the member in a manner that helps the member achieve the highest possible level of independent functioning. The Behavioral Health Service Facilitator takes on additional duties within the Family Functional Therapy (FFT) program by providing family sessions using the FFT structured model for treatment

Essential Functions and Responsibilities
The following duties are primarily performed and are essential for this position.  Employees are expected to be able to perform each of these job duties satisfactorily and successfully with or without reasonable accommodations to be qualified for the position.  Other duties may be required and assigned.

35% Ongoing Assessment:
Provides ongoing assessment of mental status and symptomatology. Works within each consumer’s recovery team and facilitates services based on everyone’s recovery goals. Educates individuals and their families regarding symptomology for them to live in the least restrictive setting and have maximum access to the community. Ensures all consumers are offered services in the following categories based on their individual needs and goals:  screening and assessment, service planning, service facilitation, diagnostic evaluation, medication, physical health monitoring, peer support, individual skill development and enhancement, employment related skill training, individual/family psychoeducation, wellness management and recovery/recovery support services, psychotherapy,
substance abuse treatment. Facilities family sessions in accordance with the FFT model

30% Psychosocial Recovery Planning and Implementation:
Ensures that each consumer’s recovery plan and service delivery is integrated, coordinated, and monitored, and is designed to support the consumer in a way that helps the consumer to achieve the highest possible level of independent functioning. This includes: facilitates the assessment process upon an admission of a consumer to the CCS, ensures that the consumer understands the option of using the formal or informal grievance resolution process, directs the provision of services as identified in the consumer’s service plan, being a member of the recovery team, coordinates and consults with all service providers identified in the consumer’s service plan. Participates in regular staffing with the FFT team for ongoing supervision and support while implementing program modules




10% Maintain Records:
Maintains case records per DHS 36 and Medicaid guidelines pertaining to items #1 and #2 above, as well as other records required by DHS 36 including educational, psychiatric, legal, financial, and all contracted service providers involved with the consumer. Uses multiple electronic health record systems to document cases for FFT

10% Advocacy:
Provides support and education to the families, significant others, and recovery team members of the consumer. Advocates for the mentally ill and addicted individuals in the community and provides community education

5% Crisis Intervention:
Provides crisis intervention and cooperates with law enforcement, hospital staffs, family and significant others as appropriate

5% Consultation/Training:
Participates in clinical consultation, supervises in-service training to maintain standards, certification, and personal licensure

5% Other:
Performs other duties as required/assigned

Minimum Education Qualifications

Education and/or Experience Requirements:
  • Masters degree in a behavioral health related field (social work, counseling, psychology, etc) with training license (APSW, LPC-IT, or LMFT-IT) or permanent credentials (LCSW, LPC, LMFT) required
  • Must meet the requirements for Medicaid reimbursement
  • Minimum 2 years’ experience working with children and adolescents in a behavioral health setting is preferred

Licenses, Certifications, and Other Requirements:
  • After hire, attain Comprehensive Community Services Training Certification
  • After hire, complete Children’s Long-Term Services Certified Functional Screener program
  • After hire, complete Adults Mental Health/Substance Abuse Disorder Certified Functional Screener program
  • After hire, attain Crisis Intervention Certification
  • After hire, complete Regulation Review/Comprehension training for WI DHS 34-36--51-55-92-94-American with Disabilities Act
  • Maintain licensure by completion of 30 hours of approved CE in the first full biennium after they are licensed- 4 of the CEUs must be in area of Ethics and Professional Boundaries related to the license holder’s area of practice.  8 hours of ongoing annual training related to scope of practice also a requirement to maintain CCS and Crisis credentialing
  • Engage in all trainings, staffings, and continuing education as required for FFT designation




Minimum Knowledge, Skills, and Abilities Qualifications

  • Strong verbal and written communication skills
  • Valid Wisconsin driver’s license and liability insurance
  • Ability to use vehicles for work related duties according to the agency’s insurance liability standards
  • Ability to accurately perform record keeping and documentation entries.
  • Working knowledge of computer applications preferred

  • Prolonged periods of sitting and working on a computer.
  • Periodic periods of standing or walking.


Work Environment
 
  • General office environment with option for hybrid model (remote)
  • Ability to lift up to 20 lbs
  • Community and in home setting during client visits
  • Local travel as required fulfilling essential functions of the position
